Eligibility Requirements
Edenbridge PACE can enroll individuals who meet the following eligibility requirements:
- 55 Years of age or older
- Be determined to need the level of care required for coverage of nursing facility services
- Reside in the Edenbridge PACE service area
- Be able to live in the community at the time of enrollment without jeopardizing his/her health or safety based on criteria set forth in the Enrollment Agreement

Costs of Service
Edenbridge PACE is designed to provide coordinated healthcare, social engagement, medication management, transportation, in-home assistance, and other specialty care options.
All eligible individuals age 55 years of age and older are welcome to participate and we can help determine your qualifications for Medicare and Medicaid and assist in the application process. A private pay option is also available. Participants may be fully and personally liable for the costs of unauthorized or out of network services.

…you won’t pay a monthly premium for PACE. - IF YOU DON’T QUALIFY FOR MEDICAID, BUT YOU HAVE MEDICARE
…you’ll be charged a monthly premium for PACE. - IF YOU ARE NOT ELIGIBLE FOR MEDICARE OR MEDICAID
…you can pay for PACE privately.
