Edenbridge PACE in Washington, DC uses a combination of medical, emotional and support services enhanced by personalized care, so seniors may stay safe, comfortable, and healthy at home.
The Program of All-Inclusive Care for the Elderly (PACE) provides a specialized focus on managing the care and needs of the older adult through concentrating on the values and personal autonomy of each participant.
The PACE interdisciplinary team (IDT) guides all aspects of participant care through the development of an individualized therapeutic supports and
interventions to allow participants to remain in their own homes and communities as long as possible.
Edenbridge PACE is a health care and service support program designed to assist seniors and older adults. With Edenbridge PACE’s assistance, participants can stay in their own homes and communities. The program offers care and services in a way that supports the unique needs and preferences of each individual. Edenbridge PACE provides, coordinates and oversees all needed preventive, primary, acute, and long-term care services.
Our Services
Edenbridge PACE provides a range of services twenty-four hours a day, seven days a week, three hundred sixty-five days a year.
